  Applications are only being accepted for SLP/CCC, SLP/CF, SLP Asst, OT, COTA, PT, PTA, or Audiology Positions. Applications from individuals with one of these credentials will be kept on file for six months.

  1. After Adobe Reader has been installed, open a new tab and enter "about:plugins" in the address bar.
  2. In the list of plugins, find "Chrome PDF Viewer". If it is enabled, click the "Disable" link.
  3. In the list of plugins, find "Adobe Acrobat." If it is not enabled, click the "Enable" link.
  4. If you are already viewing the online application before Adobe Reader is enabled, you will need to refresh its page.
  1. After installing Adobe Reader, move the Adobe Reader icon in the "Applications" folder to the "Dock"
  2. Open any pdf file
  3. In response to the message "Adobe Reader is not currently configured to be the default appplication for pdf files. Would you like to make pdf files open with Adobe Reader instaed of Preview?", click "Yes"
  4. If you are already viewing the online application before Adobe Reader is enabled, you will need to refresh the page in Safari.

  If you are using Chrome or Apple/Safari you can complete the form and print it without enabling Adobe but your information will not save or be submitted unless you are using an Adobe form. Chrome and Apple/Safari are incompatible programs and will not allow your data to be saved to your SOS profile or submitted to SOS on line
